Employee Services is at SM调教所 Boulder on Oct. 25, Oct. 27 and Oct. 30 for all SM调教所 Boulder employees and spouses. Students and dependent children are not eligible.
SM调教所 Boulder students can receive a at the Wardenburg Health Center, Health and Counseling Clinic at Village Center and the Recreation Center Wellness Suite until April 2018.
Members and covered spouses of the following SM调教所 Health Plan medical plans are covered by their plan and will receive a vaccine for free:
- SM调教所 Health Plan 鈥� Exclusive
- SM调教所 Health Plan 鈥� Extended
- SM调教所 Health Plan 鈥� High Deductible
- SM调教所 Health Plan 鈥撎齂aiser
Members and spouses must bring their SM调教所 Health Plan card to receive the vaccination. Non-SM调教所 Health Plan members can receive a shot for $28.
